• Locker rental is only $5 per month plus processing fee. *
• Sign up today and your card will be charged monthly until you cancel. 
• Option to choose your locker number.
• If you are a "dual member" and want lockers at both locations, just go through the sign-up process twice. 
• Cancellation is easy! Just contact the gym staff and they will stop the fees. 

*Please note that a notice will be placed on lockers with locks that are not current on fees with a warning that items will be removed by a certain date. Sign-up today and your locker rental will auto-renew each month.

By completing the online form to process and submit my request, I acknowledge I have read and understand the locker rental terms and conditions outlined above for the Wake Forest Baptist Health Fitness Center.  I understand that locker rental is continuous from the time I sign up and until official cancellation and that the Fitness Center/s reserve the right to reject or cancel any locker rental request at any time without reason. I agree for my card to be charged monthly for the continued use of locker storage. I further understand that the purpose of the locker rental is for my convenience to store personal items. No item prohibited by WFBH policy is permitted to be stored in the fitness center lockers at any time. Locker rental dues ARE NOT REFUNDABLE.
